The role of monocyte chemoattractant protein-1 in orthotopic liver transplantation.

2003

Hepatic ischemia reperfusion injury as well as acute graft rejection (RE) after orthotopic liver transplantation (OLT) are associated with leukocyte invasion of the graft. Local synthesis of chemokines is a key reaction in the recruitment and activation of inflammatory leukocytes and consequent liver damage. In this paper we describe the role of monocyte chemoattractant protein (MCP)-1 (CCL2) in human OLT. We investigated the serum CC-chemokine levels for MCP-1 by specific ELISAs after OLT in 105 human liver allografts between September 1997 and January 2001. One hour after reperfusion we saw a significant (t test) increase of MCP-1 in peripheral blood (92.5 +/- 85.8 pg/mL to 774.2 +/- 319.…

Prevention of ischemic-type biliary lesions by arterial back-table pressure perfusion

2003

Abstract Ischemic-type biliary lesions (ITBLs) lead to considerable morbidity after orthotopic liver transplantation (OLT). The exact pathogenesis is unknown. We tested the hypothesis that insufficient perfusion of biliary arterial vessels might be responsible for ITBLs. This could be prevented by improved perfusion techniques. Since February 2000, we performed a controlled study using arterial back-table pressure perfusion (AP) to achieve reliable perfusion of the biliary-tract capillary system, which may be impaired by the high viscosity of University of Wisconsin solution. We retrospectively analyzed 190 OLTs performed between September 1997 and July 2002 with regard to ITBLs. One hundre…

Brain-core temperature of patients before and after orthotopic liver transplantation assessed by DWI thermometry

2018

To assess brain-core temperature of end-stage liver disease patients undergoing orthotopic liver transplantation (OLT) using a temperature measurement technique based on the apparent diffusion coefficient of the cerebrospinal fluid in the lateral ventricles. The study group was composed of 19 patients with a model for end-stage liver disease (MELD) score of 23.7 who underwent MR imaging before and after OLT. MR imaging studies were performed with a 1.5T MR scanner. Brain-core temperature (T: °C) was calculated using the following equation from the apparent diffusion coefficient (D) of the cerebrospinal fluid in the lateral ventricles: $$T = {{2256.74} \mathord{\left/ {\vphantom {{2256.74} {…

Long-Term Results After Liver Transplantation With “Livers That Nobody Wants” Within Eurotransplant: A Center's Experience

2008

Abstract Background Orthotopic liver transplantation (OLT) represents the only curative treatment for end-stage liver disease, but its application is limited because of organ shortages. The purpose of this study was to review the long-term outcomes after OLT during a 2-year period of 45 rescue offers organs within Eurotransplant. Patients and Methods Forty-five deceased donor liver allografts had been officially offered to and rejected by other transplantation centers 162 times prior to our acceptance. Data analysis addressed recurrence of primary disease, ischemic-type biliary lesions (ITBL), re-evaluation or relisting for OLT, re-OLT, as well as overall patient and graft survivals. Result…

Der Einfluss des RANTES −403 Genpolymorphismus auf den Langzeitverlauf nach orthotoper Lebertransplantation

2005

Introduction: By the recruitment of leukocytes chemokines are able to mediate a protective as well as a harming influence on rejections after human orthotopic liver transplantation. There is a base exchange (G/A) known at position-403 in the RANTES promoter region which leads to an increased RANTES expression. Due to the crucial influence of a raised hepatic RANTES synthesis on the course after liver transplantation we investigated the effect of the RANTES −403 polymorphism on organ survival and rejection frequency. Materials and methods: Genomic DNA has been extracted from EDTA-blood of liver transplanted patients. The RANTES −403 polymorphism has been analysed by hybridisation with FRET (…

Brain Core Temperature of Patients Before and After Orthotopic Liver Transplantation Assessed by DWI-Thermometry

2019

Purpose The brain produces heat as a result of cerebral metabolism, and the heat is removed mainly through circulation of the intracranial blood vessels and cerebrospinal fluid (CSF). The aim of this study was to assess the brain core temperature of adult patients with end stage liver disease before and after orthotopic liver transplantation (OLT) using a noninvasive temperature measurement technique based on the diffusion coefficient of the cerebrospinal fluid. Materials and Methods This retrospective study used the data collected from January, 2014 to January, 2017. The study group comprised 19 patients (16 men, 3 women, mean age 57.9 ± 7.4 years) with a MELD score 23.7 who underwent magn…

Metabolomics discloses donor liver biomarkers associated with early allograft dysfunction

2014

Background & Aims Early allograft dysfunction (EAD) dramatically influences graft and patient outcome after orthotopic liver transplantation and its incidence is strongly determined by donor liver quality. Nevertheless, objective biomarkers, which can assess graft quality and anticipate organ function, are still lacking. This study aims to investigate whether there is a preoperative donor liver metabolomic biosignature associated with EAD. Methods A comprehensive metabolomic profiling of 124 donor liver biopsies collected before transplantation was performed by mass spectrometry coupled to liquid chromatography. Donor liver grafts were classified into two groups: showing EAD and immediate g…

Comparison of two different techniques of reperfusion in adult orthotopic liver transplantation

2006

: The aim of this study was to determine the impact of two reperfusion techniques on the peri-operative hemodynamic changes and early post-operative graft function of adult patients undergoing orthotopic liver transplantation. Material and methods: From June 2003 to May 2004, 50 consecutive liver transplants were performed and divided into two groups: group A, 25 patients, portal vein flush with 500 cm3 of Ringer's lactate without vena caval venting. Group B, 25 patients, vena caval venting with no portal vein flush. Donor and recipient characteristics were similar in both groups. Sixty-four different parameters were analyzed, and Pearson's χ2 test and t-test were used for statistical analy…

Acute and reversible Pisa syndrome as unusual presentation of portosystemic encephalopathy

2020

Abstract We present the first case of acute and reversible Pisa Syndrome, as a clinical manifestation of a portosystemic encephalopathy bout occurring in a patient affected with cirrhosis and clinical-radiological signs of acquired hepatocerebral degeneration, without exposure to psychotropic medications. A 62 year-old man suffering from cirrhosis was admitted to our hospital for sudden onset of mild confusion and postural change. He was observed walking and standing with a tilt toward the right during the last two hours. On neurological examination, he showed bilateral asterixis and extrapyramidal signs. Clinical diagnosis of Pisa Syndrome was made in a setting of raised serum ammonia. A b…
